如何为在中国出生的儿童申请第一张旅行许可证或签证

　　Pleasepay attention to the following contents when handling the firstcertificate of a child (born in China).
　　办理儿童（出生在中国的）第一次证件，请注意以下有关内容。
　　I.Conditions for handling affairs
　　一、办事的条件
　　1.Both parents are foreigners;
　　1.父母双方均为外国人；
　　2.One parent is a Chinese citizen and has registered residence in thiscity.
　　2.父母一方为中国公民且具有本市户籍。
　　II.Procedures
　　二、办事的手续
　　(i)Application for foreign visa or residence certificate if both parentsare foreigners:
　　（一）父母双方均为外国人的申请外国人签证或居留证件：
　　1.Submitthe original and copy of parents＂ passport;
　　1.提交父母护照原件及复印件；
　　2.Submit the original and copy of the child＂s birth medical certificatein China (including the duplicate page);
　　2.提交儿童在中国的出生医学证明（包括副页）原件及复印件；
　　3.Originaland photocopy of children＂s valid foreign passport;
　　3.儿童有效外国护照原件及复印件；
　　4.If the parent works in Shanghai, submit the original applicationmaterials of the work unit;
　　4.如父或母在沪工作的，提交工作单位申请材料原件；
　　5.Submit the completed Alien Visa Application Form and a recent 2-inchhalf-length full-face bareheaded photo;
　　5.提交填写完整的《外国人签证证件申请表》、一张近期2寸半身正面免冠照片；
　　6.The parents＂ marriage certificate (can be issued by the relevantcivil affairs, health, public security and other governmentauthorities, notary departments or the embassy (consulate) of thecountry to which they belong, issued by foreign institutions, must becertified by our embassy (consulate) abroad, and non-Chinese onesneed to be provided with a Chinese translation issued by a domesticqualified translation agency);
　　6.父母的结婚证书（可由相关的民政、卫生、公安等政府主管部门、公证部门或所属国驻华使（领）馆出具，系国外机构出具的，须经我驻外使（领）馆认证，非中文的需提供国内有翻译资质机构出具的中文翻译件）；
　　Suchchildren must come to apply for a foreigner visa or residence permitwithin 60 days after birth.
　　此类儿童须在出生后六十天内前来申请办理外国人签证或外国人居留证件手续。
　　(ii)If one of the parents is a Chinese citizen and has the registeredresidence of this city, they can apply for the Exit Entry Permit ofthe People＂s Republic of China
　　（二）父母一方为中国公民且具有本市户籍的，可申请《中华人民共和国出入境通行证》
　　1.Submit the original and copy of the child＂s birth medical certificate(including the attached page);
　　1.提交儿童的出生医学证明（包括附页）原件及复印件；
　　2.The original and photocopy of the child＂s foreign passport;
　　2.儿童的外国护照原件及复印件；
　　3.Valid identification of parents;
　　3.父母的有效身份证明；
　　Chinesefather (mother): submit the original and copy of the city＂s residentID card and the city＂s household register;
　　中国籍父（母）：提交本市居民身份证、本市户口簿原件及复印件；
　　Foreignfather (mother): submit a valid foreign passport (if the original isChinese, the original and copy of the registered residenceregistration cancellation certificate must also be provided for thosewho later join the foreign register);
　　外国籍父（母）：提交有效外国护照（如原为中国籍，之后加入外国籍的还须提供户籍注销证明原件及复印件）；
　　4.The parents＂ marriage certificate (can be issued by the relevantcivil affairs, health, public security and other governmentauthorities, notary departments or the embassy (consulate) of thecountry to which they belong, issued by foreign institutions, must becertified by our embassy (consulate) abroad, and non-Chinese onesneed to be provided with a Chinese translation issued by a domesticqualified translation agency); For those who have children beforemarriage or have children before marriage, the parent-childidentification report of the child and its parents issued by China＂sjudicial identification agency must be provided;
　　4.父母的结婚证书（可由相关的民政、卫生、公安等政府主管部门、公证部门或所属国驻华使（领）馆出具，系国外机构出具的，须经我驻外使（领）馆认证，非中文的需提供国内有翻译资质机构出具的中文翻译件）；未婚先育的或先育后婚的，须提供我国司法鉴定机构出具的该儿童与父母亲子鉴定报告；
　　5.Submit the completed Application Form for the Certificate of Entryand Exit Pass of the People＂s Republic of China, and threehalf-length full-length, bareheaded color photos with a smooth faceof 2 inches (48 * 33MM) on a white background.
　　5.提交填写完整的《办理
　　Theprocedures for such children must be handled by both parents.
　　此类儿童的手续须由其父母双方来办理。
　　Note:If the exit and entry administration of the public security organdeems it necessary to interview the applicant or request to submitother supplementary materials, and fails to accept the interview orsubmit the materials within the agreed time, except for the reasonsof force majeure, it shall be deemed to be cancelled.
　　注：公安机关出入境管理机构认为必要时需要面谈申请人，或要求提交其它补充材料，除不可抗力的原因外，未在约定时间内接受面谈或提交材料的，视为撤销办理。
